Consent form for persons participating in the E-Learning and Leadership Program


  1. I consent to participate in this program, the details of which have been explained to me, in the project Fact Sheets.
  2. I understand that after I sign and return this consent form it will be retained by the National Rural Women’s Coalition Ltd. (NRWC)
  3. I understand that my participation in the e-learning and Leadership program will involve online discussion groups and that this work will be recorded and some of these recordings may be shared with other participants, for example those unable to attend the live classes.
  4. I acknowledge that:
    1. I have been informed that the confidentiality of the information I provide will be safeguarded subject to any legal requirements;
    2. I have been informed that class sessions will be taped and may be retained on the NRWC Website;
    3. I give permission for photographs in which I appear to be used for the publications and public relations activities related to this project. In particular, I note that this may include use in print and electronic media, including the Internet.
  5. I agree to have my name and contact details shared with other participants in this program for the purpose of group work.
  6. I acknowledge that NRWC will not share participant names, individual work or personal information details with those not participating in the program.
